Application for Equine Structural Integration Sessions

Fields marked "*" are required while unmarked fields are optional

ex: trailering accident, slip and fall in pasture, crash on a jump course, herd aggression, incidents in training sessions, environmental factors like fireworks, pollution, violence from humans (including threats of violence), nutritional deficiencies, etc.
Please check all that apply
Irregular tooth wear, gum disease, tooth decay, oral tumors, inflammation of the mouth, lip/cheek lacerations, etc.
Please list how long between trimming/shoeing, barefoot or shod, and any issues with hoof care.
Please list when saddle was last fit and any details about current or previous saddle fit or indicate if horse is unridden.

(acupuncture, chiropractic, etc.)
ex: Colic surgery, joint injections, insulin resistance, ulcers, cryptorchidism, laminitis (any rotation?), etc.

Turnout schedule, stabling, number of feedings/day, herd environment, blanketing, etc.
Please note any imbalances, patterns, tendencies such as "energetic" vs "lethargic", etc.
